Qualified Intellectual Disability Professional 0222N (Joliet, IL)
Location: Joliet, IL
Salary Range: $30,000 - $33,000/annual
Schedule/Shift: M-F, 8:00-4:30pm
Performs highly responsible duties related to the development, implementation and coordination of Personal Plans for individuals with intellectual disabilities. Completes assessments; maintains, reviews and evaluates pertinent program data relative to each individual's growth and development; guides and leads the Community Support Team in treatment and habilitation methods; ensures that services are being correctly implemented; provides training to direct care staff as indicated; reviews outcome progress on a monthly basis. Assists families in the Home Based Support program with monthly budgeting, personal support worker enrollment and timekeeping, and maintaining accurate records. Serves as the point of contact between participants, service coordination representatives and fiscal intermediary agents. Conducts home visits as requested and documents activities to create monthly billing notes. Assists with various other agency wide supports as needed.
Bachelor’s degree in a human service field and at least one year experience working with individuals with developmental disabilities. Additional qualifications include: balancing, pulling, pushing, standing, climbing, grasping, stooping, crouching, crawling, kneeling, reaching, lifting, repetitive motion, pushing wheelchairs, and lifting up to 40 pounds
